What should I bring?
- Identification documents for your I-9:

- A photo ID (e.g. driver’s license, passport, or employment authorization card all work well) AND
- A document showing you’re authorized to work in the country (e.g. social security card, birth certificate, passport, authorization card, etc.).
- See the I-9 form here – see page 3 for a list of acceptable documents.

- Your best self! You will have your picture taken for your ID badge, website, etc.
Where is the HR office located?
We are located at 130 E. 9th Street on the north side of John C. Fremont Park. Park in the roundabout and enter via the south doors. They are access controlled so let them know you’re here to see Kate in HR. HR is located inside the first door on the left at the top of the stairs.

New Hire Forms

Benefit Forms - MUST Return to HR
Required Forms – These will be given to you at your in-person appointment.
- *Blue Cross Blue Shield Enrollment Form – This form is required. If you enroll in health or dental insurance, you’ll need to complete pages 1, 2, and sign on page 3. If you decide to decline both health and dental insurance, you only need to complete section H on page 3.
- *Election/Declination Form – This form is required and tells the business office what you’re choosing to do regarding health insurance. This form is completed by everyone each year.
- *Met Life Enrollment Form – This $10k Life Insurance Policy and Long Term Disability is provided to all Certified employees at no cost to the employee. We are required to have this form on file.
- *Voluntary Life Insurance Enrollment Form – You are eligible for up to 5x your annual income, up to 100k, in voluntary life insurance coverage without having to provide evidence of insurability. If you decline during your initial enrollment period (when you’re hired), there is open enrollment every fall. However, the maximum employees are allowed to sign up for is 20k during open enrollment. So it will take five years to build your coverage up to what you are eligible for during your initial enrollment period.
- *NPERS Beneficiary Form – This form requires a Notary for your signature. Kate Baker, the HR Coordinator, is a notary! Be sure to make an appointment when dropping off your paperwork and we can get your form notarized for you. Please don’t sign it prior to arriving for your appointment.
Optional Forms
- New Teacher One-Time Salary Advancement Form – If you wish to opt for a salary advance to help with expenses in August, the deadline for this form is August 9. A check can be made out to you as early as August 1st.
- Vision Enrollment Form
- Vesting Credit for Retirement (NPERS) – If you have been enrolled in another Nebraska state retirement plan, complete this form and follow the instructions regarding submitting it to the NPERS office.
- FPS Foundation Donation Form – This form is completely optional and is included here as part of the FPS dress code. If you would like to ability to wear jeans every day, you can in exchange of a small monthly donation to the FPS Foundation. $15 minimum for jeans day every day.
- YMCA Enrollment Form (return to Kristyn Drey, our Payroll Associate) – FPS offers payroll deduction for YMCA members. FPS employees also have their joiner fee waived by the YMCA. To join, complete the form at this link and return it to the Payroll office. We will share it with the YMCA. You will still need to go to the YMCA for a membership card and facility information.
