Your Health and Wellness

As an FPS employee, your health and wellness is our priority. If you:

  • are experiencing symptoms of COVID-19 and seeking a medical diagnosis or
  • are subject to a Federal, State, or local quarantine or isolation order related to COVID-19 or
  • have been advised by a health care provider to self-quarantine due to concerns related to COVID-19 or
  • are caring for an individual who is subject to an isolation/quarantine order or who has been advised to self-quarantine due to concerns related to COVID-19 or
  • are caring for your son or daughter if the school or place of care of the son or daughter has been closed, or the child care provider of such son or daughter is unavailable, due to COVID-19 precautions

Please read through the information below and complete STEP 1 (on the right). You will be contacted by HR as soon as possible once this is received.

Grounds for Emergency Paid Sick Leave (EPSL):
The amount of sick pay, if you qualify for EPSL, is based on the reason for your leave and your pay. The FFCRA provides up to a total maximum of ten work days of emergency paid sick leave to each employee to the extent that the employee is unable to work for one of the six reasons listed below:

  1. The employee is subject to a Federal, State, or local quarantine or isolation order related to COVID-19
  2. The employee has been advised by a health care provider to self-quarantine due to concerns related to COVID-19
  3. The employee is experiencing symptoms of COVID-19 and seeking a medical diagnosis
  4. The employee is caring for an individual who is subject to an order as described in (1) or has been advised as described in (2).
  5. The employee is caring for his/her son or daughter if the school or place of care of the son or daughter has been closed, or the child care provider of such son or daughter is unavailable, due to COVID-19 precautions
  6. The employee is experiencing any other substantially similar condition specified by the Secretary of Health and Human Services in consultation with the Secretary of the Treasury and the Secretary of Labor.

I have two or more symptoms of COVID-19, what do I do?

A member of my household has tested positive for COVID-19, what's next?

  1. Notify your administrator and Human Resources
  2. Complete Step 1, if you have not already done so.
  3.  Send documentation of the positive test to Human Resources, kate.baker@fpsmail.org

I've tested Negative, can I come back to work?

If you have tested negative, you can return to work as long as:

  1.  Your symptoms have improved
  2. You have been fever free (below 100 degrees) for 24 hours without medication
  3. You have not been exposed to another person who has tested positive to COVID-19 within the last 14 days

Do I have to present my Vaccination Card to Human Resources?

Yes. Please send an image (via email or fax) to Katie Plank at kathryn.plank@fpsmail.org. If you know the date of your second vaccination and it’s not on the card, please share that information with Katie as well.

I have symptoms but I have been vaccinated. What do I do?

  1. Notify your Administrator
  2. Complete the HR COVID-19 Notification Form

Even if you have been vaccinated, if you are having symptoms we need to determine what those symptoms are and we need to know when you were vaccinated. Once we have that information, we’ll be able to advise on next steps.

I've had a close contact exposure to a known positive case but I have been vaccinated. Do I need to do anything?

Even if you have been vaccinated, if you have had a known exposure, there are certain precautions employees will be required to take.
